Medical Records Clerk
General Summary of Duties: Responsible for a variety of tasks including collecting patient information, issuing medical files, filing medical records, and processing requests as assigned.
Examples of Duties: (This list may not include all of the job duties).
- Gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and physicians' offices; retrieving information from computer/fax.
- Issues medical files to persons and agencies according to laws and regulations.
- Distributes medical charts to the appropriate departments/individuals.
- Maintains quality and accurate records by following clinic procedures.
- Ensures patient charts, paperwork, and reports are completed in an accurate and timely manner.
- Handles requests for records and documents for billing or legal services, sending and receiving information via fax machine, as assigned.
- Makes sure all medical records are protected and kept confidential.
- Files all patients' medical records and information.
- Completes clerical duties including answering phones, responding to emails, and processing patient records/requests.
- Duplicates documents and forms as necessary.
- Generates invoices.
- Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.
- Serves and protects the clinic community by adhering to professional standards, clinic policies and procedures, federal, state, and local requirements and standards.
- Enhances professional growth and development through in-service meetings, education, conferences, etc. Attends required meetings and participation as requested.
- Adheres to patient confidentiality protocol.
- Performs other duties as assigned.
Education: A high school diploma or equivalent is required.
Experience: Typing ability of 35 wpm. Computer experience required.
